introduce an optional QUOTE configuration line. handle quoted fields within CSV files, and the possible presence of delimiters with quotes fields.

Introduce an optional HEADER configuration line. Based on a patch from Freda Bundchen, but heavily tweaked by me. This makes some comments redundant, delete them from the config files. Also simplify -I handling while here and improve related error handling.
