In private header files, __BEGIN_DECLS and __END_DECLS are pointless.
Because these work slightly differently on different systems,
they are becoming a maintenance burden in the portable version,
so delete them.

Besides, one of the chief design goals of the mandoc toolbox is to
make sure that nothing related to documentation requires C++.
Consequently, linking mandoc against any kind of C++ program would
defeat the purpose and is not supported.
I don't understand why kristaps@ added them in the first place.

Delete the __attribute__((__bounded__(...))) annotation.
That's an OpenBSD-specific gcc-4.2.1 security extension.
It's certainly a bad idea to use such stuff in a compatibility header,
as other operating systems just won't understand it.

Merge from OpenBSD - Marc Espie improved the ohash interface:
* rename the halloc callback to calloc, provide overflow protection
* rename the hfree callback to free, drop the useless size argument
* prevent integer overflows in ohash_resize

whitespace and comment changes, no functional change

Use C99 uint32_t, not BSD-style u_int32_t.
Needed for Solaris 10 as reported by Matthias Scheler.

Add a compatibility interface for ohash.
This include's espie@'s wholesale src/lib/libc/ohash directory from OpenBSD
into compat_ohash.c (with a single copyright/license notice at the top)
and src/include/ohash.h as compat_ohash.h.
The ohash_int.h part of compat_ohash.c has been changed only in that ohash.h
points to compat_ohash.h.
Added HAVE_OHASH test (test-ohash.c) to Makefile.
In mandocdb.c and mansearch.c, check HAVE_OHASH test for inclusion.
